Stressed proteins can cause blood clots for hours

Mar. 5, 2013 ? New research from Rice University, Baylor College of Medicine (BCM) and the Puget Sound Blood Center (PSBC) has revealed how stresses of flow in the small blood vessels of the heart and brain could cause a common protein to change shape and form dangerous blood clots. The scientists were surprised to find that the proteins could remain in the dangerous, clot-initiating shape for up to five hours before returning to their normal, healthy shape.

The study -- the first of its kind -- focused on a protein called von Willebrand factor, or VWF, a key player in clot formation. A team led by Rice physicist Ching-Hwa Kiang found that "shear" forces, like those found in small arteries of patients with atherosclerosis, cause snippets of nonclotting VWF to change into a clot-forming shape for hours at a time. The finding appears online this week in Physical Review Letters.

"When I first heard what Dr. Kiang's team had found, I was shocked," said blood platelet expert Dr. Joel Moake, a study co-author who holds joint appointments at Rice and BCM. Moake, whose research group was the first to describe how high shear stress could cause platelets to stick to VWF, said, "I had thought that the condition might last for such a short time that it would be unmeasurable. No one expected to find that this condition would persist for hours. This has profound clinical implications."

Kiang, associate professor of physics and astronomy and of bioengineering, studies the forces involved in protein folding. Proteins are the workhorses of biology. Tens of thousands are produced each second in every living cell, and each of these folds into a characteristic shape within moments of its creation. Despite its ubiquity, protein folding is an immensely complex process that is shrouded in mystery.

Kiang is a pioneer in the use of atomic force microscopes (AFM) to shed light on the fundamental physical processes involved in protein folding. The AFM has a tiny needle with a tip measuring just a few atoms across. The needle is suspended from a tiny arm that bobs up and down over a surface. Kiang's team uses the bobbing needle to grab and pull apart individual protein molecules. By stretching these like rubber bands, her team has shown it can measure the precise physical forces that hold them in their folded shape.

"In this study, we did more than just measure the forces; we used those measurements to see what state the molecule was in," Kiang said. "In this way, we were able to study the dynamics of the molecule, to see how it changed over a period of time."

Moake, a senior research scientist in bioengineering at Rice and professor of medicine at BCM, said the work is vitally important because it helps explain the workings of VWF.

"VWF is synthesized in the cells that line the walls of blood vessels, and it's stored there until the cells get signals that the vessels are in danger of injury," Moake said. "In response to those stimuli, the cell secretes VWF. It's a long protein, and one end remains anchored to the cell while the rest unfurls from the wall like a streamer."

The act of unfurling makes VWF sticky for platelets, and that begins the process of hemostasis, which prevents people from bleeding to death when blood vessels are damaged by cuts and wounds.

"The body recognizes when clotting must stop -- when there are too many strings, too much sticking, too many platelet clumps -- and it uses an enzyme to clip the long VWF strings," Moake explained. "First, it makes large, soluble versions of the strings that remain somewhat sticky, and then these large soluble portions of VWF are reduced into smaller subunits of VWF that circulate in the plasma."

Under normal conditions, these circulating subunits, which are called PVWF, fold into compact shapes and cease to be sticky to platelets. However, previous research had shown that a type of physical stress called "shear" -- which can arise in partially occluded arterial blood vessels with high flow rates -- could cause PVWF to become sticky to platelets.

"That's all we knew," Moake said. "We didn't know how the conformation of the PVWF protein changed. That is why Dr. Kiang's research is so important and makes it more likely that therapeutic interventions can be more rationally designed."

To study the problem, Kiang's lab worked closely with Moake's team at Rice's BioScience Research Collaborative and with researchers from the laboratory of co-author Jing-fei Dong, formerly of BCM and now at PSBC in Seattle. Moake's and Dong's groups prepared samples of PVWF, subjecting some to the shear stresses known to induce clot formation. Kiang's team used AFMs to test the samples. Through a combination of experiments and deductive reasoning, her team determined exactly which portion of PVWF changed its conformation during shear stress. They also determined how long the protein remained partially unfurled before relaxing into its natural shape.

"The next step will be to design new experiments that allow us to monitor the proteins as they bind to platelets and initiate clot formation," Kiang said. "That will tell us even more about the physical properties of the proteins and provide more clues about potential therapies."

Gibson Foundation and the Everett Hinkson Fund. Quantum realm: Forging new pathways to quantum devices

Mar. 4, 2013 ? Physicists at UC Santa Barbara are manipulating light on superconducting chips, and forging new pathways to building the quantum devices of the future -- including super-fast and powerful quantum computers.

The science behind tomorrow's quantum computing and communications devices is being conducted today at UCSB in what some physicists consider to be one of the world's top laboratories in the study of quantum physics. A team in the lab of John Martinis, UCSB professor of physics, has made a discovery that provides new understanding in the quantum realm and the findings are published this week in Physical Review Letters.

"As one crucial step of achieving controllable quantum devices, we have developed an unprecedented level of manipulating light on a superconducting chip," said first author Yi Yin. Yin worked on the project when she was a postdoctoral fellow in the Martinis Lab from 2009 to 2012. She relocated to her native China last fall, where she is now a professor at Zhejiang University in the city of Hangzhou.

"In our experiment, we caught and released photons in and from a superconducting cavity by incorporating a superconducting switch," said Yin. "By controlling the switch on and off, we were able to open and close a door between the confined cavity and the road where photons can transmit. The on/off speed should be fast enough with a tuning time much shorter than the photon lifetime of the cavity."

She explained that not only can the switch be in an on/off state, it also can be opened continuously, like a shutter. In that way, the research team was able to shape the released photons in different wave forms -- a key element for the next step they want to accomplish: controlled photon transfer between two distant cavities.

Co-author Yu Chen, also a postdoctoral fellow in the Martinis lab, said that this way of moving information around -- sending and catching information -- is one of the most important features of this research. "In optics, people imagine sending information from Earth to a satellite and then back -- really remote quantum communication," he said.

"The shutter controls the release of this photon," said Chen. "You need to perfectly transfer a bit of information, and this shutter helps you to do that."

Co-author Jim Wenner, a graduate student in the Martinis lab, explained another application. "Another one, again with communication, would be providing ways to transmit signals in a secure manner over long distances," said Wenner.

He said that, instead of another shutter, Yin used classical electronics to drive the photon. She then captured the signal in the superconducting cavity, in an area called the meander, or the resonator. Then the shutter controlled the release of the photon.

Wenner explained that the resonator, a superconducting cavity, is etched on the flat, superconducting chip -- which is about one quarter of an inch square. It is chilled to a temperature of about minus-273.12 degrees Celsius.

Yin completed her B.S. in physics at the University of Science and Technology in China, before going to Harvard University to earn a Ph.D. in physics. Of the time she spent at UCSB, Yin said: "The Martinis group is one of the best groups in the field of superconducting quantum devices in the world, which strongly attracted me to find the opportunity to work here.

"The whole group is a very young, energetic, and creative team, with the strong leadership and support of Professor John Martinis. I am very happy to have learned the advanced techniques and to have studied the exotic quantum devices of this group." She credits the support of the entire UCSB team, especially important technique support from co-authors Yu Chen, Daniel Sank, Peter O'Malley, Ted White, and Jim Wenner.

In addition to Martinis, the other co-authors from UCSB are Rami Barends, Julian Kelly, Anthony Megrant, Charles Neill, Amit Vainsencher, and Andrew Cleland. Contraception in women over 40

Mar. 4, 2013 ? Despite declining fertility, women over age 40 still require effective contraception if they wish to avoid pregnancy. A review article outlines the risks and benefits of various contraceptive options for these women. The article, based on current evidence and published in CMAJ (Canadian Medical Association Journal), is aimed at helping physicians find the best methods for their patients.

The benefits of use outweigh the risks for most contraceptive methods used by women over 40. Even for women with risk factors for complications, methods are available that can be safely used to prevent unintended pregnancy. Several methods are considered "top-tier" by the World Health Organization, including copper IUDs, progestin implants and sterilization.

Contraceptives containing estrogen should be used with caution in women over 40 who have cardiovascular risk factors, because of the increased risk of blood clots. Although some patients are concerned about an increased risk of cancer, studies have not shown a negative link between oral contraceptive use and cancer. Use of oral contraceptives is associated with a reduced risk of endometrial and ovarian cancer.

In addition to preventing pregnancy, oral contraceptives can have some additional noncontraceptive benefits for women over age 40 years, such as alleviating heavy menstrual bleeding and diminishing hot flashes in perimenopausal women.

Condoms and diaphragms may also be effective, because failure rates are lower among older women than among younger women.

"Clinical judgment will be required to balance the risks and benefits when a woman has multiple medical conditions," writes Dr. Rebecca Allen, Warren Alpert Medical School of Brown University, Women and Infants Hospital, Providence, Rhode Island, US. "The availability of safe, effective options suggests that estrogen-containing methods should increasingly be used with caution in older women who have cardiovascular risk factors."

Obama says his team managing cuts best they can

WASHINGTON (AP) -- President Barack Obama presided Monday over the first meeting of his new-look Cabinet in a sobering climate of forced fiscal belt-tightening, urging humane management of spending cuts for communities and families that are "going to be hurting."

Obama said he's continuing to seek out Republican partners to reach a deal to end the so-called "sequester," but there was no sign that a breakthrough was in the works to reverse the $85 billion in budget reductions that went into effect Friday.

"We are going to manage it the best we can to minimize the impact on American families," the president told reporters allowed in for the beginning of the meeting. "It's not the right way to go about deficit reduction."

Obama sat next to his newly confirmed Defense Secretary Chuck Hagel, with new Treasury Secretary Jack Lew across the long oval table, next to Vice President Joe Biden. Obama said his team also planned to discuss other priorities for his second term, including immigration reform, gun control and expansion of funding for preschool.

Obama's spokesman was more pointed in his comments on sequestration in a briefing preceding the Cabinet meeting. White House press secretary Jay Carney said it's remarkable some Republicans are calling the deep government-wide cuts a win for the conservative tea party, or for Republicans stood up to the president.

Carney said the cuts go against conventional GOP goals of increasing defense spending and border security. He also said the cuts do nothing to address Republican priorities for long-term deficit reduction or tax reform.

Obama "hopes that having achieved this empty victory, at least as they see it, the Republicans will understand that their goals are being unmet here," Carney told reporters. "So not only are Americans suffering from this, regular folks, but their objectives are being unmet and there's an opportunity to change that dynamic."

The Senate's Republican leader, Mitch McConnell, on Sunday called those cuts modest, and House Speaker John Boehner said he wasn't certain that they will hurt the economy.

"This modest reduction of 2.4 percent in spending over the next six months is a little more than the average American experienced just two months ago, when their own pay went down when the payroll tax holiday expired," McConnell said on CNN's "State of the Union."

"I don't know whether it's going to hurt the economy or not," Boehner said on NBC's "Meet the Press." ''I don't think anyone quite understands how the sequester is really going to work."

White House economic adviser Gene Sperling said the pain will be felt eventually. "On Day One, it will not be as harmful as it will be over time," he said.

Both parties are casting blame on the other for the automatic, across-the-board spending cuts but gave little guidance on what to expect in the coming weeks. Republicans and Democrats pledged to retroactively undo the cuts, but signaled no hints as to how that process would start to take shape. Republicans insisted there would be no new taxes and Democrats refused to talk about any bargain without them.

All of this comes ahead of a new, March 27 deadline to deal with the question of funding the government and a debt-ceiling clash coming in May.

Obama has phoned lawmakers but it isn't clear to what end, and the White House would not say who Obama is calling.

Obama and the Republicans have been fighting over federal spending since the opposition party regained control of the House of Representatives in the 2010 midterm elections. The budget cuts were designed in 2011 to be so ruthless that both sides would be forced to find a better deal, but they haven't despite two years to find a compromise.

The $85 billion in cuts apply to the remainder of the 2013 fiscal year, which ends Sept. 30. But without a deal they will continue slashing government spending by about $1 trillion more over a 10-year period.

Nearly 1 in 4 women newly diagnosed with breast cancer

A study by researchers at the Herbert Irving Comprehensive Cancer Center (HICCC) at NewYork-Presbyterian/Columbia University Medical Center, has found that nearly one in four women (23 percent) newly diagnosed with breast cancer reported symptoms consistent with post-traumatic stress disorder (PTSD) shortly after diagnosis, with increased risk among black and Asian women. The research has been e-published ahead of print in the Journal of the National Cancer Institute.

"This study is one of the first to evaluate the course of PTSD after a diagnosis of breast cancer," said lead author Alfred I. Neugut, MD, PhD, the Myron M. Studner Professor of Cancer Research, professor of medicine and epidemiology, at Columbia University's College of Physicians & Surgeons and Mailman School of Public Health, and a member of the HICCC.

"We analyzed interview responses from more than 1,100 women," said Dr. Neugut. "During the first two to three months after diagnosis, nearly a quarter of them met the criteria for PTSD, although the symptoms declined over the next three months. Younger women were more likely to develop symptoms of PTSD, and data suggest Asian and black women are at a more than 50 percent higher risk than white women."

The 1,139 research participants were part of the Breast Cancer Quality of Care Study (BQUAL). Between 2006 and 2010, women with newly diagnosed breast cancer, stages I to III, over the age of 20 were recruited from NewYork-Presbyterian/Columbia University Medical Center and Mount Sinai School of Medicine in New York City; the Henry Ford Health System in Detroit; and Kaiser-Permanente in Northern California. Each participant completed three phone interviews. The first was two to three months after diagnosis and before the third chemotherapy cycle, if the patient was receiving chemotherapy. The second interview was four months after diagnosis, and the third was six months after diagnosis.

"The ultimate outcome of this research is to find ways to improve the quality of patients' lives," said Dr. Neugut, who is also an oncologist at NewYork-Presbyterian Hospital/Columbia. "If we can identify potential risk factors for PTSD, when women are diagnosed with breast cancer, we could provide early prevention and intervention to minimize PTSD symptoms. This approach might also have an indirect impact on the observed racial disparity in breast cancer survival."

The research team believes that these findings may apply to patients with other cancer diagnoses as well. Dr. Neugut noted that in previous research, symptoms of PTSD have been reported following prostate cancer and lymphoma diagnoses.
